New bupivacaine implants (Xaracoll) will lead to questions about the safe use of local anesthetics for post-op multimodal analgesia.

Xaracoll is only for open inguinal hernia repairs. The implants are placed into the surgical site to reduce pain for up to 24 hours...then they break down and are resorbed over time.

Xaracoll will compete with liposomal bupivacaine (Exparel), which lasts about 24 hours or more...or pumps (On-Q, etc), which slowly infuse local anesthetics at the surgical site for up to 5 days.
